This is a prescription only product and can not be dispensed without a signed prescription from a physician.
Your physician can use his/her own prescription pad or use the one included with this packet.

Items to fax with your signed prescription are:
- Copy of your insurance card(s), front and back
- Medical notes from a visit to your physician that verifies and gives details about your diagnosis
- Contact information for you (Name, Address, DOB, etc.)
